- 2 Passport size photographs of employer
- 2 Passport size photographs of employer’s spouse
- 2 photostat copies of the employer’s I/C
- 2 photostat copies the employer’s marriage certificates
- 2 photostat copies of the children’s birth certificate
- Must provide B/BE form with receipt (without resit, must provide J form or EPF statement) (Declared joint income must be above RM36,000 for Indonesia Maids and RM60,000 for Filipino Maid)
- Employer’s and spouse latest 3 months pay slip
- If the residential address is different from the I/C, you have to submit either original electricity bill, water bill or telephone bill
- If you are the director of the company, you should provide company registration letter.
- If you are applying maid to look after patient or aged parents, documents needed as below :-
- Patient’s I/C
- Doctors letter
- Documentation that can proof the relationship of the employer and the patient, such as employer’s birth certificate
